Ask your fiance to have a medical check up to find out whether there is a risk for him with sex.
When a fat and a non-fat person have sex, they usually prefer intercourse positions with the non-fat person on top.
Obesity itself is of course a large risk factor for many diseases. But getting rid of obesity will only be successful if the obese person understands the risk and wants to get rid of obesity.
